Gerald “Jerry” Joseph Chouinard, 74 of Claremore, Oklahoma passed from this life on Thursday, November 28, 2019 in Salina, Oklahoma. He was born January 23, 1945 in Vallejo, California to his parents, L. J. “Frenchy” Chouinard and Mary Elizabeth Karr Chouinard. Jerry married the love of his life, Carol Ann Creighton on June 18, 1965 in Dallas, Texas and they have shared in over 54 years together.Jerry is survived by his loving wife, Carol Chouinard of the home; two sons, Mark Chouinard and his wife Stephanie of Collinsville, Oklahoma and Shawn Chouinard and his wife Jackie of Collinsville, Oklahoma; one brother, John Joseph Chouinard of Jefferson City, Missouri; two sisters, Shirley Maddux of Milford, Texas and Pamela Parker and her husband Dan of Midlothian, Texas; six grandsons, Austin, Tyler and Matthew Pittman, Tyler and Landon Chouinard and Cody Butler; one granddaughter, Destiny Butler; and two great-grandsons, Ryker Houtman and Kolter Pittman. He was preceded in death by his parents; one son, Chris Chouinard; and two sisters, Ellen Chouinard and Rose Mary Chandler.Visitation will be held from 3-7 p.m. on Tuesday, December 3, 2019 at Floral Haven Funeral home. Services will be held at 12:30 p.m. on Wednesday, December 4, 2019 at Floral Haven Funeral Home Chapel with Pastor James Lacey officiating. Burial will follow at Floral Haven Memorial Gardens, Broken Arrow, Oklahoma.
