Survived by wife, Donna , daughter Stella, and son Scott. There are two brothers, Billy Ray Maddox and Jimmy Maddox surviving as well as numorous neices and nephews. There has been a Memorial Fund started , those wishing to donate in lieu of flowers, donations to Jerry Maddox Memorial Fund, at any Bank of Oklahoma location.
